Community Access

Staying connected to your community is fundamental to wellbeing. First Priority Care's community access support helps NDIS participants get out of the house, attend activities, build social connections and participate in the community life they want to live.

What our Community Access include

  • Transport to and from appointments, activities and community venues
  • Support to attend social groups, clubs and community events
  • Accompaniment to sporting and recreational activities
  • Support to access educational courses or vocational training
  • Assistance with shopping, banking and everyday community tasks
  • Support to build social skills and community connections
  • Cultural and religious activities and participation
  • Support to attend and participate in family events

Who is this service for?

  • Participants who are isolated or at risk of social isolation
  • Participants who want to try new activities but need support to do so safely
  • Participants transitioning from school into adult community life
  • Participants with complex communication needs who benefit from a familiar support worker when in the community

How we deliver it

Community access support (also called Social and Community Participation under the NDIS) is funding for a support worker to accompany you into the community. This might be getting to appointments, attending a social group, participating in sport or recreation, visiting friends and family, or exploring new interests and activities.

Community access is funded under Core Supports: Assistance with Social, Economic and Community Participation (NDIS Registration Group 0125). Your support coordinator can help ensure this is included in your plan at your next planning meeting.
