Many NDIS participants live with ongoing health conditions alongside their primary disability. Conditions like diabetes, epilepsy, COPD and cardiovascular disease require consistent clinical monitoring, medication management and timely intervention to prevent deterioration. First Priority Care's registered nurses provide ongoing chronic disease management in your home, coordinating care across your health team and keeping you stable and well.
Conditions We Manage
- Diabetes (Type 1 and Type 2): blood glucose monitoring, insulin administration, hypoglycaemia management, foot care, HbA1c monitoring and diabetic wound care
- Epilepsy and seizure disorders: seizure monitoring, rescue medication administration (midazolam, diazepam), seizure diary management and carer education
- COPD and respiratory conditions: oxygen therapy monitoring, nebuliser administration, breathing assessments and respiratory education
- Heart failure and cardiovascular disease: fluid balance monitoring, weight monitoring, medication management, symptom escalation
- Renal disease: fluid management, electrolyte monitoring and coordination with renal teams
- Parkinson's disease: medication timing management, swallowing monitoring and clinical reviews
- Multiple Sclerosis: relapse monitoring, fatigue management and nursing support for MS-related complications
- Mental health comorbidities: psychotropic medication management, liaison with mental health teams
Health Monitoring Services
- Vital sign monitoring: blood pressure, heart rate, oxygen saturation, temperature
- Blood glucose monitoring and insulin administration
- Weight and fluid balance monitoring for cardiac and renal conditions
- Clinical observation and assessment with documented care notes
- Early deterioration detection and escalation to GP or hospital as needed
NDIS Funding
