Supporting Participants and Families
Palliative care is not just about the participant. It is about the entire family and care network. Our nurses take time to prepare families and support workers for what to expect, answer questions honestly and provide practical guidance so that caregivers feel supported, not alone. We also help families navigate the medical and practical tasks that arise at end of life.
What Our Palliative Nursing Includes
- Pain and symptom assessment and management: working with your palliative care team and GP
- Subcutaneous medication administration: pain relief, antiemetics, anxiolytics via syringe driver or injection
- Syringe driver management: setup, monitoring and troubleshooting
- Wound care and pressure injury prevention in end-of-life care
- Oral hygiene and comfort care
- Breathlessness management: positioning, fan therapy, medication support
- Nutrition and hydration support: including end-of-life nutrition guidance
- Emotional support and communication with participant and family
- Family and carer education: preparing carers for what to expect
- Coordination with palliative care teams, hospital in the home and GP
- After-hours support and escalation pathways
NDIS Funding
Palliative care nursing is funded under Core Supports (Assistance with Daily Life: High Intensity) and Capacity Building (Improved Daily Living). In some cases, additional funding can be accessed through the Palliative Care Support Pathway or through the My Aged Care system if the participant is approaching 65. Your support coordinator can help navigate funding.
